DEFENDANT SENTENCED IN SWEEPSTAKES TELEMARKETING SCAM Las Vegas--Attorney General Frankie Sue Del Papa announced today that Dax Gordon Dekeado, 29, today entered a guilty plea to one felony count of failure to register as a telemarketer in connection with charges of defrauding an elderly New Jersey woman out of nearly , 000. Dekeado was indicted by the Attorney General's Bureau of Consumer Protection in December 1999. Pursuant to a previous plea agreement, Clark County District Court Judge John McGroarty sentenced Dekeado to 33 months in the Nevada Department of Prisons, but suspended the sentence and placed Dekeado on three years probation. Dekeado was also ordered to pay full restitution of , 978 to his elderly victim. As a condition of his probation, Dekeado must also perform 16 hours of community service during each month of his probationary term. Between January 11 and January 27, 1999, the 73-year-old victim was contacted by telephone on five separate occasions and told that she had won a large sum of money in a sweepstakes. She was then advised that before she could collect her prize, she had to wire money to Las Vegas, via Western Union, for payment of taxes on the cash award. During this time, the victim sent amounts ranging from , 443 to , 240 to Las Vegas, but never received the promised sweepstakes money. "Consumers should never send money to telephone salespersons who claim they are prizewinners, " Del Papa said. "Legitimate sweepstakes companies never require prizewinners to send money to collect their prize." Because induction mortality is so high amongst older patients with AML, much attention has focused on the role of the haemopoietic growth factors, G-CSF and GM-CSF, in reducing the period of neutropenia after induction chemotherapy. Although these agents do appear to reduce duration of hospitalisation and febrile episodes, no consistent beneficial effects on CR rate or duration, or on OS have been seen Rowe et al. 1995; Lowenberg et al, 1997; Goldstone et al, 2001 ; . However their use following induction chemotherapy may be reasonable when it is desirable to reduce hospitalisation or antibiotic usage Ozer et al, 2000 ; . The optimal post-remission therapy for the older adult with AML in CR remains unclear. There does not appear to be any benefit from high dose cytosine arabinoside containing consolidation regimens Stone et al, 2001 ; . Moreover, given the high toxicity of intensive consolidation chemotherapy, it is difficult to justify this treatment in the older patient. In the EORTC HOVON study Lowenberg et al, 1998 ; patients attaining CR received one further course of the drugs used for induction and were then randomised to receive low dose cytosine arabinoside or no further treatment: there was a marginal improvement in DFS for those given low-dose cytarabine, but no difference in OS. There does not seem to be a role for extended consolidation treatment Goldstone et al, 2001 ; or for maintenance with either low-intensity chemotherapy Rees et al, 1996 ; or interferon Goldstone et al, 2001 ; . Aggressive salvage chemotherapy is also very seldom appropriate for elderly patients in relapse, unless the patient has good performance status and has had a prolonged first remission. In a recent phase II study Sievers et al, 2001 ; of 142 patients with relapsed AML, gemtuzumab ozogamicin, was infused at a dose of 9mg m2 on days 1 and 14: the overall response rate was 30%, with half of these patients achieving standard CR, and many of the other responders meeting all criteria for CR other than a platelet count of 100 x 109 L. Further trial work is addressing the role of this agent. However caution must be exercised with its use following recent reports of cases of veno-occlusive disease in patients treated Giles et al, 2001 ; . Recommendations 1. For patients in whom intensive chemotherapy is deemed justified e.g. age 70 years, good performance status, WBC 100 x 109 L, no adverse cytogenetic abnormalities or MDR expression ; , standard induction chemotherapy with daunorubicin or an equivalent anthracycline ; for 3 days plus cytarabine for 7-10 days is recommended Grade A recommendation, level Ib evidence ; . Where possible patients should be entered into clinical trials.
